It is extremely important that we get BOTH waiver forms from all participants for Livin The Dream and LVHN Sports Academy. If you click the links above you can access both of the waiver forms via PDF. Waiver forms must be signed by a parent or guardian if the participant is under the age of 18.
ALL PARTICIPANTS MUST HAVE BOTH WAIVER FORMS ON FILE OR THEY WILL BE UNABLE TO PARTICIPATE/ATTEND OUR EVENTS.
Option 1: Print off the waiver forms. Sign it and email it back to us.
Option 2 : Print off the waiver forms, Sign it, and bring it in/send it in with your teen for the next event.
